President Bush's father's name is George H.W Bush and President Bush's name is George W. Bush hence The "Dubya" for his middle initial.

Because W. is spelled Dubya. While watching GWB on TV, I've suspected he's had Dub ya his speeches and his body.

Because, in the south, we pronounce "W" like "dubya".

Especially in Texas.

And people called him "W" because if they called him "George Bush", he would get confused with his father.
