There is very little insurance available for fertility treatments. Brown & Brown from Texas have some insurance products (see website below). I confess I never used them, so I really don't know what it costs and their website doesn't really say.
Also, some states in the US mandate that companies provide insurance for infertility. I've included those states in the second website below. This is how my infertility treatment were covered. My state (IL) mandates that companies with more than 50 employees headquartered here MUST provdie infertility insurance. Honestly, we didn't know that when we moved here, so that was a very pleasant surprise.
Other than that, fertility treatments are usually out-of-pocket and quite expensive. Your (or spouse's) employer may provide insurance to determine the cause of the infertility, but not the correction; this happens in a lot of cases.

Fertility coverage is determined by the plan you have or the company you work for and sometimes by the state where you reside.
I had fertility insurance through Aetna and through Blue Cross Blue Shiled but my company, based out of Massachusetts was an enormous corporation required by the state to cover fertility.

You need to have insurance before pregnancy or you'll be consider as a pre-existing condition and have to pay everything out of pocket. Can you get insurance through your job? I have Blue Cross and Blue Shield. If you have a low income you can apply for Medicaid. You need a free quote to see how much you will have to pay monthly for insurance. Look on line if you have to for insurance company's.

Good luck finding one that pays for fertility treatments.
We have great insurance, but it doesn't cover them.
Call agents and ask to know for sure who carries the plans.
