Besides checking for lice in your pubic hair as suggested by someone earlier, maybe you could see if the problem is due to : a)ingrown hairs on your scrotum, if you are in the habit of shaving scrotum hair;
b)see if the condition is due to wearing too tight underwear and/or due to wearing underwear made of synthetic material. In both cases, sweat tends to accumulate on your skin, including on the scrotum, and generates itch due to bacterial action.
Try wearing cotton underwear, change it daily, wash your pubic area including pubic hair, if you have retained it, with normal non-medicated soap, and sleep without wearing underwear.

Maybe craps. But that's a long time of crabs. Check your pubic hairs for any small white particles attached. If so, it maybe crabs eggs or crabs themselves attatched to your hair (a form of lice for the crotch area). If you can see them then either see your doctor or go to the nearest rite-aid or thrifties and buy you some crab and lice remover. Some products are for both crabs and lice, and some are just for lice.

Are you Brazlian? Because if you are, they have naturally dirty itchy nuts, and nothing you can do will get rid of it. I'm sorry. You'll also smell like taco seasoning when you sweat, and their females lay eggs, much like frogs.
