How does someone change their religion, especially if it's a Mormon religion, where they don't leave you alone? How does someone write a letter regarding this to confirm their decision to be removed from their religion list? What if the Mormon church refuses, or ignores a request, then what? Can an attorney be involved?

Religion is a personal choice that no one should argue with you about. You shouldn't require a letter.

However, if it is part of the Mormon rules that a formal letter be written, address it to the head of your church. Just state that you wish to leave the church and don't wish any further contact with them.

If they refuse to leave you alone, you can report them to their superiors as well as the police. Harassment is against the law.

If they still refuse to leave you alone and won't accept your decision, by all means, consult an attorney, providing proof that you have made a formal request to leave the LDS church. And note times they've called or come to the door, names if possible and anything else which might help.

You could look on the bright side. You have a group of people that feel their belief is the correct one and would like to bring you into the fold to save your soul. Isn't it nice that they care so much about you. You do have the right to disagree and if they become unbearable, you can file a restraining order against them.
