Can someone provide an example of what I should write in the ebook to prevent theft. I don't mind people issuing the book to their customers for free, I just don't want someone stealing the content and passing it as their own.

Do I need to do a disclosure on every page. Anybody have an example?

If it's electronic you can't stop someone from lifting the text out of it and removing your disclaimers. If someone is going to steal it then they will ignore your request not to steal it... as they are not moral anyway.

You can state your copyright on the text, but plagiarism is known even on paper based books...

You could prevent any text from being copied/pasted.
