Forget Freddy Kruger carving up Johnny Depp, OK>
This has got to be the scariest piece of filmmaking i have EVER seen, as we are watching this guy slowly killing himslef.

For me, the scariest movie I've ever seen was The Shining. I still love that movie to this day!

For me, the original Halloween starring Jamie Lee Curtis. I still have nightmares seeing Michael Myers (or "the shape" as he is known in the credits) standing there looking up at her window. She notices him, flinches, and when she looks again he is gone.
Another is "The Entity" starring Barbara Hershey about a woman who is continually being raped by a ghost. The movie itself isn't really too scary, but the fact that the movie is based on a true story is scary stuff.

The Exorcist, because it focuses on characters and the slow development of dread. It does not rely on JUMPS and LOUD NOISES to startle you. It sets the stage and builds the atmosphere in a realistic way.
There is a big difference between a startling film and a terrifying film.
