It seems like no matter what settings we try with the defroster, the windows and windshield fog up like crazy! The inside of the windshield also frosts occasionally. Why does this happen? How can we fix it?

i own a repair shop,and there are two possibilities on that,one would be the heater core leaking on it,,and another one would be you have a leak in the car,,and its letting rain water in,,and when it gets cold the glass will draw the moisture right to it,,you might want to check the carpet,,and see if it feels damp,or wet,,if it is you will need to find the leak,as long as there's any water in it at all,it will do this,,and a bad heater core,will leave a greasy like film on the windshield,,if you don't have this,,you probably have a small leak in the car,,maybe around a door or window,,it don't take much water to cause this,,good luck,i hope this help,s.

Mild Irritation is correct, there is a pin hole either in your heater core (a minitrature radiator) under your dash or a pinhole in a hose going to it.
When your engine gets hot it makes steam which fogs up your windows. eventually youll get low on water in your radiator and the car engine will overheat

Turn on the AC button, it gets the humidity out of the air. Keep the heater on, just turn on the AC button. Otherwise it'll fog up.

You have a leak in your heater core. Hot water from the engine is making steam inside your car.

Turn on A/C while on defroster. This should blow AC on the windows, and yes it worked even in freezing weather in NY.

You have water inside Your car(may be You put the carpets without drying them.)

get ,somehow, dry the inside of Your car.

check the heater core for leaks!!!!

P.S. if heater core leaks smell sweet.inside Your car.(coolant smell)

Looks like you should make a trip to your local Toyota dealer and get that fixed.

sounds to me like you got a heater core leaking.if thats what it is,then all you can do is by-pass it(which you wont have any heat)or change the heater core.
