We don't have sensors attached to TV so how???? I need help

The Neilson Ratings commission takes a survey. They take selection of viewers and put a monitoring box on their TV. The results are averaged for the population. They can also provide information on which demographic is watching which show.

AC Nielsen company uses a monitor attached to your TV. You are asked if you would like to participate. It usually numbers 1500 people. They try to get a good cross section of the population by region, age, number of people in a household and income. You are paid for your service. They take the results and multiply by the total population. This is not exact of course but it does give a fairly good representation.
