Look for French cuffs in the dress shirt department. Some shirts have the extra hole and can be buttoned or used with cuff links.

I love French cuffs! Sometimes even the catalogs will have one- I've gotten some from Chadwicks over the years.

The cuffs you seek are called French cuffs. Not very common for women, unless you buy mens' dress shirts. I would search ladies clothing sites for "french cuffs".

You need to go to the finer men's stores. Or places like Nordstrom's. Your tuxedo shops carry these shirts too.
