2) because of the heat in the atmosphere (and these have no relation with the global warming, it may be an incentive but there were hurricanes even before the industrial age) and the need of the clouds to rain
3) in large oceans and big seas, commonly in the tropic, like Atlantic ocean or pacific ocean...
4) Hurricane Katrina, Wilma, Emily, Edward....
5) really for more you could be prepared you'll never prevent the catastrophic events, but well, is not a sudden event so i suppose you always can take your car and go far away from the city and take all things you need, build in higher places (to prevent inundation) and do exactly what authorities tell you helps a lot

hmmm... someone forgot to do their homework it seems.

Your best and fastest solution is to go back and review the newspapers dealing with Hurricane Katrina. (The one that wiped out most of the gulf seaboard).

The sidebars and supplemental articles will answer every single one of your questions. You can do this on line and very quickly.
