Depends on whether you consider it a mistake. You may not realize the correlation between the action and the consequence yet, there may be too many situation in between that change your point-of-view. You may not realize you are in a similar situation until after you've already made the mistake. Or you may not know how to change the situation if you get a second chance at it.
There is also one more possibility I can think of. In all three previous situations I mentioned, eventually at some point you will realize you may still be able to affect outcome (albeit some at a point too late but still you see it). But I may point out that it is impossible to go against one's own personality. You may be in a situation in which there is an answer that will work to resolve the situation, but that you are incapable of coming up with that answer because your mind is literally incapable of coming up with the thought (whether because it is inexperienced, lacks the vocabulary to formulate the thought, or doesn't understand the process its trying to correct and so cannot formulate any theories to try out on the problem). It is quite possible that situations we have failed to correct happened solely because we didn't know what was necessary (or our mind was incapable of using the available information correctly) to fix the problem.

Because most people are ignorant and stubborn and largely selfish. They will not adimt their mistakes because they think it will ruin their image, or they are admitting to something that they absolutely hated. but most of all, they can't see what they did wrong because in their eyes, it was right
