I think it depends on what you are using the sketchbook for and your personality. Is it for a class, a personal thing, visual journal, etc? Are you more of an orderly, traditional person, or do you like experimenting and having things a little more chaotic?
I draw on both sides frequently. If the pages are thin, then I am careful not to use anything that will bleed through to the other side. Also, I'm careful to use less pressure in this case and will use a stiff paper behind the page so my drawing won't emboss the other pages. I just cut out a piece that's the size of the sketchbook and keep it in the back of the book for when I need it. If the pages are thicker then there's more freedom with what medium you want to use.
Even when things bleed through or emboss, I don't let it bother me. For me, a sketchbook is meant for playing with ideas. There's no rule that you can't use the paper (or your medium) in unconventional ways. I would even suggest drawing in all directions of your sketchbook. Turn it upside down or sideways. Tear our pieces, glue and tape things in there. Don't feel that it has to be done a certain way. Let your creativity flow.

yeah usually. a sketchbook is usually used for practice and collecting ideas. it's not usually used for a final piece, only practice for the final piece. so why waste space?
art has no limits. do what comes natural to you and you'll be right.

Hey delphinium,
Well this depends. If it is truly practice, then yes. But if you intend the images to be scanned or viewed by someone, no. Also, there is the consideration of your budget for purchasing paper, and staying environmentally green. So, use the backside when you can, but don't ruin a great piece!

Yes, sometimes.
The other day I ran across these before and after sketches here - kind of interesting:
