It was never explained. I think it was because she was brought up in a very strict home and taught to call everybody “sir”. Peppermint Patty is very much the opposite, an undisciplined girl (as you can tell from the way she does so poorly in school.) Patty never bothered to correct Marcie. She probably thought she just preferred to call her “sir”, and Marcie probably wonders if Patty really likes her or if she will ever give her permission to call her by her first name.

I don't think they ever explained it. Patty and Marcie met at camp, where Patty was Marcie's camp counselor. She called her "sir" right from the start. Patty just always rolled her eyes and thought of Marcie as a weird kid.

Pepperment Patty sounds like a dude.

Patty probably smacks her around behind closed doors and tells Marcie to call her that.
