TOEFL requirements depend solely on the university to which you are applying. Most universities require TOEFL scores from applicants whose native language is not English regardless of their immigration status. Some universities waive this requirement in cases where the applicant has a university degree from an English speaking country.

As others have said, it depends entirely on the institution to which you apply. The University of North Texas does not require TOEFL for Green card holders, but if they have a low level of English proficiency and do not feel they can succeed in university studies, they can voluntarily go through our Intensive English Language Institute. For students who have completed US high school, the SAT or ACT scores are much more important. For those applying to Graduate school, we need their GRE or GMAT scores to make an admissions decision.
At UNT, our goal is to ensure students have every opportunity to succeed. To that end, we have over 15 different measures of English proficiency of which TOEFL is only one. We also offer the Institutional TOEFL every month to give people even more chances of pursuing their dream of attending college and university classes. Other schools require the TOEFL of everyone who was not born in the US while still other schools require the TOEFL for everyone who did not earn a US or UK Bachelor degree. Your best bet would be to pick your schools and check out their website to see if they have their requirements posted.

for studying in the state, there has nothing to do with a US green card or not.
for living in English speaking countries longer than2 years, you don't need a English test in public, but they may ask you to take a school english test.
for applying any unveirsity or college, you are required to have a high school graduated or proof that you are qualified to enter school. SAT is much popular in the state when you apply the college.
Tofel is always helpful if you apply school.

no. as the abbreviation suggests, its the test of english as a foriegn language not for the native speakers.
if the green card holder hasnt studied in the US and wants to study i think it might be necessary depending whether he/she can show considerable work experience so that they can waive off that requirement.
