It means that the blank form can be seen by anyone. However, when it is filled in the form cannot be seen by any random person. Instead access to the completed form is restricted to certain people.
This is probably because the filled in form contains sensitive details which could be used innapropriately if it was available to any random person.
It is quite often used when filling out security clearance forms or other governmental forms or possibly forms with things such as banking details etc.

One assumes that the perramitres of the question will be left open if unanswered, but that if answered, some implications will be inforced.
E.G. Have you ever had a HIV test
in your scenario no answer infers no suspicion. Whilst answering yes, regardless of the results of the test, infers you are in a high risk group. Otherwise youwouldn't have been tested.
but beware, forms often contain an honesty clause so think carefully before you fudge it.

Depends what the form is for - but it sounds like it means the forms circulation or distribution is limited once it has been populated with a person's information and personal details.
