I guess only PAN card is the main thing that you need. Other things include an account in the bank with whom you want to open the demat account and also a cheqie book of the same bank. I recently opened a demat account in ICICI bank and it cost me around Rs.850/- for its creation.

Some banks are offering Dmat account facility. To open this account, Permanent Account Number (PAN) is compulsory. Also you should produce proof of residence (Passport, Election Card, etc.) along with the form.

Go to ICICI.com and fill out the forms to open a Demat account.
Take your securities that you may have in certificate form with you. They will convert it to Demat if allowed for conversion.
You will need a few things like PAN card, Mobile bill, Electric bill etc. The ICICI rep will tell you.

there are two depository service providers in INDIA .They are NSDL & CDSL under these two service provider there are many depository participent like NAVIA markets ltd , icici securities IDBI bank etc you can start a d mat account with any these depository participants . you need to provide an address proof as well as an identification proof with photography and your pan card to open the a/c . DP will charge an annual maintainence charge for providing the service .

Go to your bank and ask them the procedure for opening a demat account. These days most banks are offering brokerage services. You can also go to ICICI bank.
