It's a hard thing to do sometimes.
If I need to say something that I might not want to, I dig deep inside for the right words. This would be true in a case of being truthful to a friend even if it might hurt their feelings. I need to tell them the truth, but I don't want to because they might take offense. It is times like these that I let my heart and mind conjure up the best way to get a message across.
Speaking from your heart and telling the truth is always the best way to be sure of what you are saying.

First, disengage brain. 'Cause the confusion is probably coming from the brain thinking a mile a minute and not even stopping to take a breath because brains don't breathe the get their oxygen from the blood which picked it up from the lungs while inhaling the good air and exhaling the bad without even thinking about good and bad air or inhaling or exhaling.
Take a couple of deep breaths. Hold the second one, just a little before you exhale.
Calm your nerves and your emotions. Take a warm bath if necessary. Insense or pleasant smelling candles might help. A cup of warm tea. Avoid caffeine or alcohol.
Center youself and open to your heart. Ask yourself if you really need to say something or just thought you needed to say something.
The two extreme options are usually "I love you" and "Get lost you creep." Rule these out before going any further.
If you just thought you needed to say something but really do not, then forget the whole thing, take a big bolt of booze and smile. Job well done.
If you really do need to say something but still have no idea as to how to get the something into words then shut your eyes, shake your head, take a big bolt of booze and start talking. You might want to be in contact (phone or in person) with the person that you needed to talk to first. If it one of those long distance thingees then write the words, don't talk them.
If all else fails, you can always use the shaggy dog approach by letting your brain take back over and just let the words come out, any words, it doesn't really matter what they are, until the other person says something like "just what are you trying to say" at which point you can get huffy and tell them that if they really knew you and were sensitive to your needs that they would already know what you are trying to say and save you the embarasment of having to go through this whole thing. What I really want to say is not what I need to say, no matter how I say it. That is to say, If I think you are saying what you truly believe you are saying, then I have no words to describe how confused I am. Say what? If you knew then what you think you know now, but will not know in the future, and the past is a distant memory when you are living in the here and now, will you really know anything at all? Metaphysically speaking, of course.

I just blurt out whatever I'm thinking and try to put it together at least a little bit so it's coherent....when I'm talking about myself I tend to trip over words and not find the right ones to express what I want to say, I've always been that way...

Ahh a trick question, i like these. So be
Blunt, short,direct, and to the point.
with a provision to alter, change your mind, and or question ones own sanity at a later date...
that should cover all major "Faux Pas" should there be one

I think it over several times in my head and imagine the situation, eventual reactions and the most proper approach: making your own movie with your own script- it's fun and useful, try it out:)
