It may or not be a mental issue. Is the person on drugs? Is the person really stressed out about something else? Or maybe they have done something really bad that are are paranoid someone has found out. If not really stressed or on some type of drug (meth, cocaine, etc.,), or guilt, then
they should see a doctor. Some people really are scared they are being watched. But, if it is a mental illness, then others need to realize it's a medical condition causing it and it's not that person's fault and they shouldn't be judged.

If this person is a teenager, I would consider this behavior absolutely normal. Teenagers experience the phenomena known as "imaginary audience" in which they feel there is a hidden or not so hidden audience watching everything they say and do and often criticizing them, too. They feel like everyone is paying attention to them. This may not be too far-fetched since teens are overly critical of others and thus think others judge them the same way (and its true if they are around other teenagers).
Look at:

for more information
Also, it can carry into the early 20's and beyond if the person doesn't experience growth in their personality.

I actual have claustrophobia and tension, yet very severe..that's a affliction in my case. i think particularly uncomfortable each and each time i'm in a room with human beings i don't know, or human beings i'm not on the area of. i'm getting this in tests, instructions, gazing performances, social gatherings and any amassing the place i'm being locked in or that's silent. i especially won't be in a position to STAND silence. i'm getting so uncomfortable that i purely might desire to flee the room, if i don't, i will finally end up laid low with a panic attack while i'm out of the room because of the fact of each and every of the emotions i bottled up interior.

yes this is definetely a mental issue.. it could be simply paranoia.. or it could be a symptom of a developing bi polar disorder, it could be severe anxiety.. i would suggest going to a doctor immediately if you feel that this problem is so big it affects your daily activities..
