Yes, it can be harmful to the baby, but she needs to get in touch with a health professional ASAP, and make sure she tells them that she has a heroin problem. There's proper protocol to follow when this happens.
She also needs to decide whether or not she wants to carry the pregnancy to term. With her drug problem, she should definitely review all her options.

A friend of mine went through this. She was a user for 10+ years and suddenly became pregnant. She ended up going to a center where she lived for most of the pregnancy. They gave her methadone in regulated doses to help her cravings for the drug and counseled her so she could get off the drugs when the baby was born. If you know someone who is dealing with this, I'd suggest seeking the help of a drug abuse counselor. They can help.

Yes thre are chances for the baby to have a lot complications.
Better to seek advice from doctor first before conceiving. If she has already conceived, stop all form of drugs and seek doctor advice all the time. Best to monitor the baby's growth in womb all the time.

She should NOT stop using heroin abrubtly. She needs to see an OBGYN so that she can start methadone treatment to help her withdrawl slowly. She will need to be monitored closely during her pregnancy, provided she wants to carry the baby to term.
