First of all, there are no power supplies with a fan on top. The "top" fan you are talking about is the bottom fan, that brings air into the power supply. And do yourself a favor, and buy one with two fans. They not only run alot cooler, they also help keep the rest of your system components quite a bit cooler.

Also, a single fan power supply MUST be run in a case with positive pressure*, or they overheat. Well, that's assuming that you're planning on doing something other than web browsing, e-mail, and word processing/spreadsheets.

*Positive pressure means more air coming into the case than going out. In most cases, it means you'll have to cut a hole in the side of your case, so you can add an extra fan.

It just means that the fan is inside the PSU, this design helps reduce the noise you will hear.

It could also have 2 fans. One large on top or bottom of PSU, and the other smaller one hidden inside.

There is only a few PSU's that have NO fan at all, but those need to be installed in a well ventilated case.

I have the Antec Phantom 500W, and it only has 1 fan that comes on depending on the setting i put it at. This PSU can work without the fan spinning at all.
