Well, a golden duck is when you get out to your first ball, and a royal duck is when you get out to the first ball of the innings (see http://en.wikipedia.org/wiki/Duck_(cricket) ). Since a royal duck is a kind of golden duck, I would guess that a royal golden duck is the same thing as a royal duck, ie getting out to the first ball of the innings.

In fact it's also mentioned here (http://en.wikipedia.org/wiki/List_of_cricket_terms#D ) under the entry for 'diamond duck' as meaning exactly that in New Zealand.

The opening batsman goes out for a golden duck on the first ball of the innings – sometimes known as a black duck, diamond duck or royal duck.

If an opening batsman gets out on the first ball of the both innings that means he gets a royal golden duck.

Never heard of it. A golden duck is when you get out first delivery and a duck is when you get out for zero but not on the first ball.
