Clouds form anywhere atmospheric conditions cause the temperature to decrease to the dewpoint. Fog forms at ground level, clouds form at 40,000 feet (and higher), and every other altitude in between.

Based on height,the clouds are classified into mainly three catagories.Low clouds form between 2000 feet to 5000 feet sometimes extending upto 10000 feet.Medium clouds form between 10000 feet to 12000 or 13000 feet.High cloud form above 30000 feet .This is only a rough classification particularly in tropical countries.Actual heights may vary depending upon the latitude and topography of the place. Cumulonimbus cloud which gives thunder and lightning is a low cloud extending from approximately 1000 feet to 35000 feet.Hence clouds may form from the surface to 35000 feet in the trophosphere.

Excellent question again. clouds form anywhere in the atmosphere from near the surface to the noctilucent clouds in the mesosphere at 80-85 km. In the troposphere where most of the clouds you see form the bases of these clouds vary from around 1000 feet to near 35,000 feet.

The highest clouds are called Cirrus and exist at about 30000ft.
