Throw it away and buy a new one. It may also be too big. Go to Victoria's Secret they'll size you.

A bra riding up or moving around indicates that your band size is too big. You might want to check your cup size as well. You can get fitted at just about any department store or VS and such. If you prefer you can try yourself of have someone you know do it for you.
This is a general guideline for bra fitting and will general apply to most, but may not apply to some.
To measure for you band size, use measuring tape and measure around your ribcage just under your breasts - check to make sure the tape is level from front to back or it will measure wrong. This number is your band size. Your bra should be snug around your ribs, this is the primary support, not the straps. A brand new bra should fit on the last hooks, not the ones further in. As you wear the bra it will get a bit stretched out, that is when you adjust to a tighter hook
Next measure around the fullest part of the breast. Subtract this number from the band size to get your cup size. About every inch will equal one cup size (A=1", B=2", C=3", etc)
Also consider the material. If it feels itchy or bothers your skin, check what it is made of, maybe you'll like something else better. I love microfiber bras, they are all I own now. They are soft and comfortable and breathable. I love them more than cotton or anything else.

Get one that fits right. Bras are EXPENSIVE, at least I think so. It is really hard to find a good one, or a good brand. Some bras (especially the cheap crap at Wal-Mart) make your boobs look like pointy cones and they don't support well. The cup is long but doesn't have a big enough circumference for your chesticle. So, you gotta try a few.
Even though they are VERY VERY EXPENSIVE, Victoria’s Secret bras are the bomb. They feel comfortable enough to sleep in. You want to make sure your strap is long enough and the bra fits very comfortably around your chest. The cup size is the hardest because there really aren’t half sizes. Try a few sizes and see what does you best. I prefer my ta-tas to be close together so I buy bras that allow for that. Some are made to 'lift and separate' so you just go with what is comfortable. As far as cheap bras go (and I don't know where you are from but I'm in the Midwest USA) family dollar stores has good, cheap lacy bra/panty sets. They're only like $5 each and they are shaped well (no Wal-Mart cones). These are for your fancy sorts. For sports bras go to any big box store, like Wal-Mart, Meijer or Kmart. Sports bras don't really go by cups just by circumference so you'll want to buy one that is a larger circumference than normal if you are large breasted but small chested. It is very complicated but I hate having an uncomfortable bra so I have tried many things. I have large ones so I have to use underwires but many people don't like underwires. Bras without underwires lose shape and cannot hold large loads so they don't always work. Victoria’s Secret makes a good bra without underwires that is still just as good (don't remember what it's called, though)
If your bra fits all around but you still have the problem you need to adjust the arm straps. If that doesn't work, you might want to buy a softer one so it's more comfortable.

If your bra is at all mobile on your body you are not wearing the right size. To find the band size (the number) measure under the bust and round to the next even number. Many people also advice measuring under the arm pit over the top part of the breast but I find that gives a fuller size than I find comfortable. To find cup size measure over the fullest part of the breast generally across the nipple and then subtract the band size from that the if the difference is 1" you need an A cup, 2" a B and so on.

I've had myself sized at VS just like people are suggesting and I have the same problem with the itching. I just have to take it off the minute I get home from work.

Buy a new bra. It sounds like you have the wrong size bra. I buy Victoria's Secret bra's and they fit nicely.

If its riding up, loosen the straps. If it slides around, its too big, if its uncomfortable its too small. Buy a new one.
