Quotes are used when citing an essay or poem, but when you type up a novel italicize it, don't underline it. Underlining is only used for when you are writing with pencil and paper because it is too hard to write in italics.

No. Simply make the font 2 to 4 sizes larger than the text of the body. Do not underline.

You only do one of the above if its in your works cited page, otherwise you put the name of the piece in an introductory statement like " . . . ' as so and so said in __________. If you dont use an introductory statement you just go off the works cited and put the line or page # in ()S at the end of a sentence.
