All of the above. First grab the yellow pages.
You can call a local glass company to order new "tempered insulated units". You can have them install it or you can. If you install you will have to tell them the width, height, overall thickness of the unit (outside glass surface to outside glass surface), and the thickness of the pieces of glass (Probably 1/8" or 5/32" thick. Keep it the same so the door weighs the same.) Some places stock the two most common sizes: 34x76 and 46x76.
However, ordering 4 custom insulated units may be more expensive than buying new doors. You can get quotes for either way over the phone from various places.

Sounds like a "pane in the glass" pardon the pun..
It certainly would be nice if you could just replace the glass.
Check the glass for etching, also the hinges for a clue as to the manufacturer. Those options being exhausted, perhaps it would be prudent to consider ordering new patio doors.
Among the more reputable vendors are
1.Andersen windows
2.Kolbe and Kolbe
3.Crestline
4.Marvin
5.Jeldwen
Also, if you are considering purchasing 4 sets of patio doors, that should give you a little leverage as to a price discount.
Definitely get more than one quote, and get quotes from qualified carpenters as well, if your not doing the installation.

Depending on the age and condition of the doors, it may be less expensive to replace them. There may be identification numbers between the panes. If there are you could take them to a home improvement store or glass company and they may be able to help identify the manufacturer, there may be a warranty.
If this fails the glass company can fabricate new glass panes for you. This won't be inexpensive so you should shop around for new doors and weigh your options. Replacement may be your best option.

To save money? You can call someone locally who sales glass, like Home Depot or Lowes. If they don't they will know who does.
You can either buy the double panes like you have now [which is more costly], or you can buy single panes and put them in to make a double. They will cut them to size, then all you need is the caulking to seal them once they are replaced in the frames.

You will have to order the replacement double-pane glass panels according to their specific size. The door companies will highly unlikely have these replacement glass panels in-stock. Call a glass contractor to measure and install.
