When I buy jeans they will fit every where but in the butt area, their always a little to big and sag down, I don't like to wear a belt and I can't go to a smaller size because then it would be to small in the waist. How and should I have them altered?

Don't do it yourself. Go to a Tailor...they know what they are doing.

You should cut down the seam in the butt area along the side. Then, measure how much you want to take off and align the jeans back to the seam so it overlaps underneath, and pin it. Since the thick seam of the jeans is too hard to sew, sew the uncut side of the seam together with the underlayer. Sorry if this sounds confusing, but do it step by step and it should work out!

Either buy a smaller size & have a tailor add on fabric somehow to the waist to make it fit OR tough it out & wear a belt. What's wrong with wearing a belt anyways?
