Clean underwear smells like cotton and fabric softener (whatever brand used). Dirty man's underwear can range anything from a musky toasted nut scent after a day to something very rancid if worn for a week (think urine, feces, etc.). If they were used to go to the gym they will smell like sweat and whatever natural scent the guy carries along with pheromones.

There are several ways to find out. First, if you have brothers just smell theirs. Second, go to the gym and swipe a pair from the locker room. Or go over to a friend's house or have them come over to yours to spend the night and sneak a sniff. Pretty easy to find out. You can also buy them on the internet apparently used underwear is a real turn on so people sell them.
