I do not earn income in Illinois other than this interest - I live in Indiana. Do I simply pay income taxes to Indiana or do I also need to file an Illinois tax form?

Interest is taxable based on residency. Just make sure you were not resident in Illinois for any part of the year. If you weren't you only have to pay tax on the interest to the state you live in.
