A: My Boss is NOT going to be happy about this

If your boss gets to work early, or stays late, do the same. Ask them if they have a moment to talk. Shut the door behind you.
Tell them how much you've enjoyed working with them and so on. Then tell that as much as you hate to, you need to resign.
Tell them that you will work with them to help train someone to take your place and that you'll stay a little longer than the necessary two weeks if it takes that long to find the right person to replace you.
Your boss will respect that you've put the company's needs ahead of your own and you won't have to worry about leaving on bad terms.

Your two week notice, eh? Talk to your boss, tell her why you are looking to leave, citing specific people/attitudes/reasons etc. Write it all down beforehand before talking.

If you want to stay and work on some of the issues, ask for a raise. A big one. Let you boss know that you won't take anything less, as you already have another job lined up and waiting for you.

Arrange a time to speak with your boss. Make a specific appointment so as not to be interrupted.
