There are several templates for resumes on most word programs. If you have microsoft word, you can go to the help/search section and type in "resume" and several optional templates will pop up. You can use the template that appeals to you the most. The templates highlight the sections that you need to complete, so all you need to do is click on each section and enter your personal info. As far as the info to include, make sure to list all of your duties/responsibilities, as well as any raises or increases in responsibility based on quality performance.

Job search websites like Monster and Hot Jobs let you create a resume by filling in some blanks. You can do that, and print out the finished result.
Microsoft Word also has some decent resume formats that you could follow.
