If you ever get the chance, read the "Peter Principle" by Dr. Laurence J. Peter. This is an older book, but it is timeless because human nature does not change that much. In effect, Dr Peter says that bureaucrats actually create more work than they accomplish, so more bureaucrats and more assistants are hired to take care of the extra work the first bureaucrats create. Sounds like a joke of some kind, but this is a serious study of the British Admiralty during a time when it was discovered that there were six administrators for every sailor in the British Navy.
The other part of the Peter Principle is that people are promoted to their level of incompetency, that is, if someone is good at a job, they will be promoted upward untill they get to where they cant handle their job any more. Then they will be kept there. Reason for this is that there is a shame in being demoted, and they wont be moved anywhere else because they appear to be incompetent.
I dont completely agree with everything Dr Peter says, but he has his basic ideas right.
I would like to point out that here that these days we pay more for administration than ever before, and get worse administration than ever before. I really do not like the idea of professional management, I E, management majors. When one these clowns gets into an organization, you have someone who does not know the product or the business particulars of the company they are making decisions for. They apply the unrealistic gobbledegook they learned in Business School, from people who are isolated behind ivy-covered walls from the real world All too often, their degree leads to an arrogance which does not let them learn anything new.
The best managers are ones who are promoted up through the ranks, and who are humble enough to know their level of competency so they do not get into the situation described in the "Peter Principle".
The "Peter Principle" should be required reading in ANY management training.

The answer is not that complicated because bureaucracy is ambiguous and no one person is really accountable for anything.
Unlike private business where if you screw up, you can be fired or go out of business. Bureaucracy on the other hand just raise your taxes when they screw up that's is why they should be limited.
