For a common or regular procedure in which the patient doesn't show an oxygen deficiency, that 'clip' is put on the person, but if there's a major procedure to be done or the patient has oxygen related problems, a larger device is attached to the person that includes the necessary oxygen to mark a normal level while working on the patient.
