Certainly high on the list would have to be Golden State trading Robert Parish and their number 3 pick (who turned out to be Kevin McHale) to Boston for the number one and 13 picks (Joe Barry Carroll and Rickey Brown). Maybe that's a bit unfair since it involves poor choices in draft picks.
Another candidate has to be when the Charlotte Hornets traded the draft rights to Kobe Bryant to the Lakers for Vlade Divac. Ouch!
Perhaps the worst trade not involving draft picks was the 1965 trade in which the Warriors sent Wilt Chamberlain to the Philadelphia 76ers for Connie Dierking, Paul Neumann, Lee Shaffer and cash. The warriors had financial trouble and needed the cash badly, but trading the greatest scorer of all-time in his prime?

I'm from Toronto, so i'm easily gonna have to say the Vince Carter deal with the Nets. This is how the worst trade in NBA history went down...

To the NETS:
Vince Carter

To the RAPTORS:
Alonzo Mourning (who refused to play for the team and got his contract bought-out, then joined the Heat to win a championship)
Eric Williams (never did anything but ride the bench)
Aaron Williams (never did anything but ride the bench)
1st Round Pick (turned out to be Joey Graham)
1st Round Pick (was traded to the Knicks)

I'll throw in another "non trade" answer - the absolute worst personnel decision EVER, hands down, had to be Portland picking Sam Bowie over Michael Jordan in the '84 draft. How'd you like to be forever known as the team that "passed" on the greatest player ever in the NBA?

Tyson Chandler for Elton Brand
Leandro Barbosa for a second round draft pick

Cant pick one....but I can narrow it down to 2.

#1 Milwaukee Bucks trading Dirk Nowitski for Robert Traylor.

#2 Warriors traded Robert "The Chief" Parrish for Joe Barry Carrol and a #3 pick...which ended up being Kevin McHale.

Stephen Marbury to the once mighty Knicks with the max contract.
Patrick Ewing trade out iof Knickss the worst trade I've seen. Not a fan of Knicks but one thing for sure Ewing is the foundation of the Knicks. Without him, Knicks crumble into what it is today.

It wasn't really a trade but in 1956, Red Auerback persuaded the owner of the Rochester Royals to pass up drafting Bill Russell. In exchange, Auerbach arranged for Rochester to get the Ice Capades which was also owned by the owner of the Celtics. Bill Russell went on to win 11 titles for the Celtics. Rochester got... a week of the Ice Capades.

ahhh man, since im a warriors fan this is to me was the worst trade or give away, heres a list

1. Wilt Chamberlain
2. Vince Carter
3. Chris Webber
and 4. Gilbert Arenas/Antawn Jamison

those people ended up to be great players, the warriors were impatient then and thats why they didnt want them, they needed players to good right away, but if you wait then these players can blossom to become good scorers and more

When the moron Maloofs traded Chris Webber to Philidelphia, then Peja Stojakovic to Indiana...they tore down one of the greatest teams ever made, my Sacramento Kings of 2001-2004. A team that could have built a dynasty had we just had better luck.
