A really cool thing to do would be to write the White House & the President will sign a birthday card for individuals who are 80 yrs old & older. You must write (they don't accept Email). The card will be sent direct to your grandmother.Send your request to:

They want your request to be sent 6 wks in advance so add to your request that you know its sent late but a belated card would still be a card she would treasure.

,,Hi! My Mum didn't live to that age. So I'll borrow your Mum ok, as a sue do Birthday Girl I can find a gift for. I know it's not easy , at that age they have lots of things and need or want ,very little. Depending on her interests and your Bank account. If it's something you want to have as an heirloom to pass down from her. Then a nice little antique brooch, jewel box, if possible an early edition Book of Poetry, painting or drawing.by some renowned artist. It's surprising just how inexpensive these items can be. However go to a reputable dealer.She'd be a Royalist, due to her age and history with the royals. So an Invitation to the "Queens Garden Party." She has 1 at least once a year that I know of at Buck House. The Public are invited. Try asking your local community information. Buck; House themselves, when and how to get an invite. A Spanish or Irish lace head scarf, that she could wear.They're beautiful but inexpensive. You could give her some of those long forgotten, framed times of days gone by, when she was in her prime. ie. copies of old magazine articles, newspaper. dated on the day she was born. Fashions, Cooking and house hold hints when she was a young wife. All that kind of memorabilia. Down size them and Place it in an album or even frame it like a collage.

Gift some thing which will be really helpful at this old age. (gift both physical and emotional pleasure)
For emotional gift :
* spend good time with her making the mood more cheerful and light. Chat with her, sit with her, eat with her, prepare her favorite dishes, present her with a good bunch of flowers
* If she is bored in the home from a long time, take her to a pleasure trip to some good pleasant place for a change
* Find out what she was asking all these days and for what you were not able to make yourself available - look for the possibility of that
For Physical comfort
* Depending on the type of health problem, gift a good spectacle / elegant hand stick / tooth / Body massager for pain relieving or some medical insurance scheme ... which ever suits
* Since they need to Pass the time, gift them a good piece of MP3 player or Audio/Video system with remote

I made my 80 year old grandmother a scrapbook of the family for christmas. Every page was dedicated to one person of the family. she LOVED it. We also got her a throw made because she always gets cold. We had a picture sent in, and they transfer that picture to the blanket, and it turned out perfect. The picture was clear and she also loved that gift. Good luck :)

A trip to a day spa, she can get a facial, get her hair done, maybe makeup, have her get dressed nice, go to the photographer, get some professional pictures done (We do it so often for our kids, but never for our parents) ....then take her out for a nice dinner.

I would make a scrapbook of your lives together from birth until now. I think she would like that more than anything store bought and it takes thought and time.

You will be surprised to hear this, but a bouquet of Roses. Many Ladies have "never" received them. I gave them to my aunt, who, unknowing to myself, had never received them and she was overwhelmed.
Go for it! and have a wonderful time with her.
