Depends on what type of incision they use.

One type of incision, the lollipop, means they cut around your nipple, then straight down to the bottom of your breast.

Another, the Anchor, cuts around your nipple, down to the bottom of your breast, then a smilie incision around the bottom half of your breast.

Yet another one cuts around the nipple, then the smilie cut.

They'll of course put you under, take out all the fatty tissue and whatever else they need to, then close you up. Again, depending on the surgeon, will depend on how they will close you up. My doctor had internal stitches (that dissolve over time) and used steristrips for the outer incisions. Some will use internal stitches AND outer stitches.
