To prevent it, don't put your mail out in a mailbox that isn't locked. People can steal your credit card statements that way - also checks you send to make payments. Next use electronic payment from your bank if you can. The payment does directly from your bank into theirs - no middle man to take the actual money.
To fix it you would have to close out the bank account, credit card accounts, etc. and open a new one. Then you would go to TRW website to place a fraud alert on your account so no one can open new accounts. TRW would alert the other two credit reporting agencies.
Also when you go out take the least amount of credit cards, etc. with you. Just take what you need.

There are actually 5 types of identity theft. I am an independendt associate for a company. We have identity theft shield. We are the only company in north america that will monitor your credit and do the restoring of your credit, if it is stolen. We will also send you an intial credit report, so you can make sure your credit has not been tapped into. We will also send you information on how to check your other types of identity (character, social security, driver's lic., and medical). If you are worried, which everyone should be, email me and I can send you information on how to protect yourself and your family. If you do decide to do it on your own, if your identity is stolen, you will get a manual over 100 pages and spend over 600 hours restoring your credit!
