You have to go to court to have it done legally, and have a "name change" court order.

Your name doesn't have to be changed by deed poll, which is really expensive. You just tell everyone of any relevance that this is the name you are now using. If you go to any solicitor they will draft you a legal name change document which (7 years ago) cost me £45. Make copies so you can give them to any organisation requiring them and keep the original safe. This should be the quickest and the cheapest way.

You don't have to go to court. All you have to do is
1. Go to DMV and have the name on your driver's license/ID changed
2. Go to Social Security office and change your last name (you will retain the same number just like you did when you got married)
3. Notify all of your creditors, banks, landlords (if any) and anyone else you get bills from of the change of last name (this can be very long though)
4. If you have children you will have to change your last name on any school records (including day care and preschool) that they have.
5. You will have to change your name on your medical/dental records as well as your children's records.
6. If you are employed you will have to notify your Human Resources department to ammend your w/4s.

And that is how you change your last name back to your maiden name.

You just inform all people dealing with you,bank,council tax office ad infinitum in writing that you are now divorced and you name is to be further known as 'insert maiden name here' the name you are known as on things is apparently the legal one. Don't forget to change passport,driving licence,credit cards etc...

make an appointment with a solicitor, it doesn't take long and the solicitor will do everything for you, give you an important "Change of Name Deed" which you get the solicitor to certify copies of to use to change all your banks etc over to the maiden name with. simpler than i thought when I did it.

If you are not yet divorced, ask for it in the divorce proceedings. If the divorce is final then you can file a petition for change of name. The court should have no problem.

Usually you do it in the divorce decree. However, if you didn't you can do a legal name change through the courts where you live.

As far as i know you need to see the people working at the town hall wherever you live, and it needs to be changed by deed poll i think. Not sure but if you ring your local town hall or civic hall or whatever they can tell you where you need to go and what you need to do

you can ask to have your formal name back when you go in front of the judge for your divorce, If you dont do it then you have to go to the court house and file to get it changed and it will cost.
