This term is basically excepted worldwide because:
The NBA is the most competitive basketball league in the world. So for a team to be the best in the NBA, it is reasonable to consider that team the best in the world. The international game may have trumped the NBA All-Stars the past decade, but the NBA comprises of players from all nations, so the term "World Champion" fits.
The NFL is the most competitive American football league in the world. (The other being the CFL). There is also the CFL, Arena FL & NFL Europe but the NFL is the league that the players in the other league strive to get to. (including Warren Moon, Jeff Garcia, Kurt Warner, etc). Therefore the term "World Champion applies here.
The same spplies to the MLB and NHL. Players from all around the world dream of playing in these leagues. These teams may play in America, but the players are representing their homelands all over the world. So again, "World Champion" works in both these instances.
MLS doe not do this, because the league recognizes that it is not the most competitive league in the world. They are trying to be, but the MLS champions are just refered to as "MLS Cup Champions."
Calling these teams "World Champion" does not mean that Americans are the best athletes in the world, as players from all nations are welcome into the league. The term applies to the team, not the player.

well, you already gave the answer to your own question about the teams that only play in America.
I suppose it would take a strong public awareness campain to pit the NBA team that wins in the us against a basketball team from a different country. But it could be done.
It comes down to money. If you could sell tickets or ad time on tv you could truly see a world champion (like the world cup for european football)

3 of 4 of those have teams in Canada. And for those sports specifically, it's not likely any other team assembled in the world could possibly beat the champs of the their league... except maybe baseball, skill matters so little in that game.

with all pro teams they recruit multi-national players for their teams, and some teams are from Canada which technically does make it international. How ever it is still a little egotistical to say world champs. Maybe it should be national champs.
