The need for surgery for a fractured talus would be determined primarily by the type of fracture, the degree of deformity and whether there was involvement of the articular surface(s).

Now the talus is a very important bone of the ankle and if you have a look at it (see the wiki page - many talus pictures there) you will see that most of it's surface is in fact involved in joints with other bones - ankle joint with tibia and fibula - talo-navicular joint (with the navicular) - talo-calcaneal joint (with the calcaneum). Therefore there is a HIGH chance you will need surgery.

However you would best discuss this with your treating orthopaedic specialist. They will be the best person with whom to discuss the risks and benefits of any treatment for your fractured talus.
