Try this website, click on the link in the middle bottom of the screen.
These nice people have a pdf of a letter to take the bank to the small claims court. By all accounts it works well.
Basically research has said that these expenses should reasonably cover the actual charge and this should be about £4.50. Where most banks take £30- £40.

Depends in general on where you are located. In some U.S. states there are local laws that limit what can be charged. There are also consumer credit laws that may be the basis for a refund. I cannot comment on non-U.S. locations. However, it is best to write a letter of complaint and start a file of copies. Obviously keep any letters of response. You want to build a history of complaints before making your demand.

Your local Citizens Advice Bureau will be able to help you out with some really good advice as to how to go about it!
