I've suffered identity theft 3 times - I think because my drivers license (with my social security number on it) was stolen when I was overseas about 10 years ago. I'm a web developer - I know how to be safe online, so I don't believe that my web behavior is the source of my stolen identity. Every time my account gets hit, it's from Turkey, Holland, or Canada - I trace the information that I'm given. I know that theives in the states steal identities and sell the info to other foreign theives, but I honestly can't think of another way that my info could have gotten circulated the way that it has.
