i am in a river and ghosts keep appearing, what am i supposed to do?!!HELP...

To add to Jamarcus' great answer, you get the Spirit Camo if you make it to the end of the river and find The Sorrow's skeleton. The Spirit Camo makes your footsteps silent, so you can run around without anyone hearing you. You also steal stamina from soldiers that you CQC.
If you use the Spirit Camo with the Stealth unit, you're almost entirely undetectable.
If you were a bad Snake, killing every soldier in your path, and you don't want to wade down the river for 40 minutes, you can always waste yourself in the very beginning and use the revival pill. You don't get the Spirit Camo, but you save some time if you're trying to do a speedrun.

the sorrow can not be killed, it is a boos that tests your will to survive. first of all, the fight is as long as you have made it. the ghost are the people you killed during the game, the less people you killed, the shorter that boss fight will be. if you killed everyone you saw, then your boss fight will last quite a while. you have to walk by the ghosts without leting them come in contact with you. if you do they will drain your health and stanima. you'll reach the end when you see a skeleton ona the sorrow will the tell you to go back to your world, when you reach the body, you'll die instantly. simply use the revivial pill that you use when you do the fake death to finish the boss fight.

You can't actually defeat The Sorrow, just run and avoid the ghosts as much as you can.

Keep walking and don't bump into the ghosts. That is all you need to do in that section. It eventually ends.
