I believe it was during the Buu Saga, after training for long amounts of time in the other dimention he gained enough power to become SS3. The dimention with the Grand Kai and fighting umm Pikon (s/p) boosted his power. And then in the GT Vegeta does become SS3 and also SS4 with help of a machine form Bulma.

I believe he became SSJ3 during one of the movies. I think it was the movie when Goku fought the giant beast and eventually killing it with a powerfull dragon uppercut. I say this because i dont recall goku being SSJ3 during the Buu Saga. But Gotenks was SSJ3 during it. And the other question. No, Vegeta used a device that Bulma made in which it skipped him a level to become from 2 to 4 during Evil shenron arc on GT.
