Go to your local SS office and take your Drivers liscence and Birth certificate with you and fill out a form for a replacement card. Makes no difference if you tell them you lost it, they will not do anything, mostly because anybody can get your number with a computer these days anyway. They can print out a temp. card and you should receive one in the snail mail from the Fed in usual Fed time 8 - 10 weeks.

Go to your local social security office with your driver's license, birth certificate and another picture ID if you have one. It takes about 2 weeks and they will mail you another one. If you know the number, it helps. Sometimes, the lines are very long, bring a portable CD player and or a book. Call 1-800-772-1213 and they can help you locate your local office. Have a great year.

Go to the SS office (call first), then you get to stand on line with people who are starting a new life with a fake I.D., it all starts at the SS Office, there you just hand in fake paper work, get a drivers lic. with it, and BAM, you have instant I.D. and a new begining, there are about 3000 people there waiting to be handed MONEY, You however, will get to fork over $15 for a new card, Thank God that I've been working for over 25 yrs straight, to have to pay for a new card, You'll love the SS office, big signs "NO FIREARMS", scuzzy people with mental problems cutting the line, while the "Square Badge" wanted felon, guard sits there and does NOTHING !

The SSA has a website which provides instructions. You can download a form and submit to your local office in person or you can mail it. You can also drop by your local branch and fill out a request. It is "no cost" if you haven't lost one in the last 10 years.
