I think that you got the basic's down just about right.
To whom it may concern:
I have known ______ for __ years. He is presently working on my horse farm as a labourer. He has shown skills in mucking out the horse stalls, brushing down the horses, blah, blah... He is a hard worker. He is reliable and applies himself well to his assigned tasks. He has shown initiative and a willingness to take on new assignments.

Start writing! Be brief, and frank, Don't try to flatter, or put a spin on it. Just be honest about what his major skills are, what his work ethic has been, and how long you have known him. Don't add anything negative, as it kind of defeats the whole purpose; let him mention those things in the interview. Keep it under a page and under four paragraphs. Use good punctuation, spelling, and grammar. It has to be readable, but not exciting.
