Is it because of intelligence, or upbringing, or just shear luck that some people make a successful life for themselves right from the start and others make mistake after mistake before they realize their missteps?

I don't look at them as mistakes. They are lessons and without adversity our lives would be horrible boring.

i don't think that you fully understand what 20/20 hindsight is.
hindsight 20/20 is for example: i should have turned the water off
to the house before i changed the faucet that way i wouldn't have this big flood in my kitchen, or i should have checked the oil in my car before going on this cross country trip maybe the engine to my car wouldn't have locked up here in the middle of nowhere, at night, in the rain and the cold, cell phone out of range.
people with common sense, the ability to predict what can go wrong (which it usually does), prepare for such occurrences.
you are in control of you're own life, what you do with it is up to you and only you (and a little luck helps ;-p).

Being decisive and sticking to your original decision are a good place to start. Don't second guess yourself. If you don't have a new, valid reason to change your decision, then don't. Many people are simply too afraid of failure to take advantage of an opportunity, which intern causes failure. These are all good leadership characteristics and can be learned. I think the most important thing you can do is have confidence in yourself. It takes a lot of courage and faith in yourself to go with your gut when everyone is going the opposite way. Realizing that the crowd is seldom right is an important concept some people continuously fail to grasp.

I think it's upbringing....
I was brought up by alcoholics, but, of course, I didn't know that...I knew they drank & were always making trouble for me... before I could accomplish anything, I had to find out about alcoholism - by then I had already made many mistakes that made my life difficult.

BUT, through Alanon, 12-Step for families of alcoholics, I started to learn to recover from the effects of the alcohol dependence of my family.....
It's a life-long process & I work on it every day...
So, luck comes into play as to who your parents are...

In Alanon I learned HOW to learn from my mistakes...it's a complex set of skills.

We don't, we have 20/20 vision. Hindsight is looking back at something that happened and making a statement like, "Well, if I'd done this then I wouldn't be in this situation now"!

It'd the classic combo environment and gentics together...you know- nature vs. nurture.....while some success is no doubt related to just being in the right place, I think the majority of success can be attributed to intelligenceand other genitic facotrs in combination with upbringing, education and other environmental factors.

We don't.

You can always look back and say "If I'd done that instead, it wouldn't have turned out the way it did" but what you can't say is that it would have turned out better. You don't know what would have happened really if you'd made any other decision than the one you made. People have a tendency to believe it's only this way or that way, when in actuality there are an infinite number of ways that anything can turn out.

I think it's that some people are good at learning from others' mistakes. Me, for instance. My sister has screwed up her life over and over again (don't get me wrong, I love her). I've learned not to make the same choices she has, and I've done pretty well for myself.

Because once you see where you have been, you try not to stumble on those same bumps in the road again...Success is in the eye of the beholder, everyone struggles. But when you conquer your own demons, whatever they may be, then you have truly succeeded!

We have 20/20 hindsight because it is better than 0/0 hindsight

we don't all have 20/20 hindsight, some people never learn from their mistakes, others might not make mistakes if they learn by watching/listening to those who erred before them, what is your question really? i'm sorry if i didn't quite answer it, i tried.
