It was Beatrix Kiddo and they used it in the second installment.
In the first she was simply known as "The Bride". That was all we needed to know at that time. When she became the kick butt assasin we found out her code name was the "Black Mamba" and that her name was Beatrix Kiddo.

They hid it when characters would outright say her name just to keep you guessing what her name was (since she was "The Bride"). But if you go back after you realize what her name is, they do refer to her many times as "kiddo" but you don't know it's her literal name until they actually reveal it.
