I have the simplest answer. IF you have another computer or another HD. If you do, then... Slave the infected HD, and run a virus scanner from master, since the Operating system is not running from the infected hard drive, none of the windows systems files that are infected are open or active, almost any anti virus will work (the reason the anti virus can not take em all out is b/c the system files are loaded into memory and can not be deleted/modfied {they are "inuse"}
IF you do not have that option, there is another way...
there is something called s.e.r.d (super recovery disk), that loads a pysdo O/S from it's cd (you boot to that cd), and you'll be able to run it from that. Serd is expensive though.
Lastly it is worth a shot, boot to dos, find your anti virus and try to run it the old fashion way,
at the c:
type CD\windows\programfiles\symantec\scanner.exe (if that is the executeables file name that is an example

Don't think anyone is making or supporting DOS based anti virus. It'd be too hard to update the virus definition files that come out weekly. Mcafee http://us.mcafee.com has the best antivirus, it knows how to remove them once they're on there. They have one for 35.00 per year, you can download it online and its a subscription service. Most others have difficulty removing them, but work great on preventing them from executing when its ran (thus preventing it from getting on the system). Have you tried Zone Labs antivirus suite? If not try to download the 15 day trial and see if its more effective ( http://www.zonelabs.com/store/content/company/products/trial_zaFamily/trial_zaFamily.jsp?dc=12bms&ctry=US&lang=en&lid=db_trial ).
But if I was in your boat, I'd try to save any data files (documents, pictures, etc) onto a CD and format the hard drive and reinstall the OS. Sounds like you've let it get way out of control.

There's a DOS scanner by Trend Micro, it's called Sysclean Package.
1. Please download http://www.trendmicro.com/ftp/products/tsc/sysclean.com (Sysclean Package) by Trend Micro and save it to your desktop.
2. Download the latest http://www.trendmicro.com/download/viruspattern.asp (Virus Pattern Files) by Trend Micro and save it to your desktop. It is named lptXXX, where XXX are numbers.
Note: Do not download the Virus Pattern Files if you don't intend to do a scan. Only download it when you want to do a scan, as they are being updated daily.
Create a new folder on your desktop.
- Right click on your desktop.
- Click on New > Folder.
- Type in Trend Micro as the name of the folder.
3. Select sysclean.com by clicking once. Press Ctrl + X simultaneously.
4. Open the Trend Micro folder you created earlier. Press Ctrl + V to paste sysclean.com into the folder.
5. Right click on the lptXXX.zip file and select Extract All.
6. Click on Browse. Navigate to the Trend Micro folder and click OK.
7. Click Next, then Finish.
8. Double click on sysclean.com to run it.
9. Check (tick) Automatically Clean Infected Files box.
10. Once the scanning is done, click Exit.

I think your best option would really be just to wipe the hard drive by formatting it and re-installing windows. I dont think there is a virus cleaner that will accuratly kill everything on your HDD.
