It does sound like the grout is coming loose, either that or you have plaster ceilings that are shedding material from a crack over that tile.

Yes, its probably the grout. It is easily fixed.

Do this. Clean the grout lines with a brush and soap and water. Then while they are still wet. Make sure they are WET. Make a mixture of regular Portland Cement and Cement hardener, mixing it to consistency of loose toothpaste. You can add cement collorant if you so chose, and go over the grout with that mixture, smooth it in there using your gloved finger in a straight line fashion. Let it harden for couple hours, keeping the room cool. The polish off the mess off of the tile surfaces with scothbrite. Wet down the repaired grout a couple times after 3 or 4 hours with a spray bottle of water. That will fix the grout guaranteed and it will also smooth up the grout so it doesnt pick up dust and dirt and turn black on you.

Dont be fooled by somesaleman at Lowes or Depot who wants to sell you some Silicone oil preservitive for grout at $35 for a few ounces. That in itself is a plain waste of money. Good Luck THE GROUT AND TILE MAN
