I will take a guess.

Self - Has to do with your conception of you, your consciousness

Identity - How you believe others should perceive you

OK I'll take a stab. Self would be characteristics we're born with... like.. gender and different tendencies that make us who we are. Identity... Identity becomes Identity when we develop our "self" characteristics... So Identity could be influenced by self, and identity is kind of who we become. I promise this sounds much more organized in my head than it does when I try to explain ! Sorry!

In my opinion identity is the ego or your personality and the self is your soul. Your "self" is constant and never changes. I think that when people go through an "identity" crisis they are seeking their true "self".
